When you are answering the Global License Information Questionnaire, you will be asked where you have formed your company. To answer this question, select the country and/or state where you filed the legal paperwork to start your company (e.g. United States - Illinois). If you formed your company in one country and/or state, and then filed paperwork to apply to do business in another country or state as a "foreign" company, select the country and/or state where you originally formed your company.
