To answer this question, please fill in the name and contact information of the individual who is ultimately responsible and accountable for your practices and policies. This is usually the individual who reviews and approves all decisions affecting privacy (e.g. a decision as to whether or not to share the personal information of customers with a particular vendor), can be contacted by privacy regulatory authorities for inquiries or complaints, and can be held responsible if your privacy practices are held to be invalid.
