Present surname: this is commonly known as the last name of an individual. For example, if the full name of an individual is “Jane Doe”, the surname would be “Doe”.
Former surnames: this is commonly known as any last names that the individual has used in the past. For example, if Jane Smith becomes married and her married name is now Jane Doe, “Smith” would be that individual’s former surname. If the Director does not have any former surnames, please leave this field blank.
Nationality: this is the country that recognized an individual as a citizen. For example, if Jane Doe was born in and is a citizen of the Republic of Ireland, her nationality is “Irish.”
